To the Editor We are concerned about Dr Trappey’s actions in treating a patient with ascites described in his A Piece of My Mind article.1

Trappey indicated that he struggled to understand the patient’s preferences and intentions with regard to his terminal condition. Yet he indicated no direct discussion of the patient’s care with either the patient or the referring physician. There was no indication that the patient’s goals and preferences were adequately clarified. His code status was not mentioned, nor was there any mention of a palliative care consult.
