In Reply We believe that the recent IDSA/SHEA guidelines on diagnosis and treatment of CDI are a significant update and provide treating clinicians with tools to better diagnose and manage CDI.1 There have been some important changes from earlier guidelines. We agree that the recommendations for treatment of recurrent CDI with either a tapered and pulsed vancomycin regimen, a standard fidaxomicin regimen, or a vancomycin regimen are based on weak data, as this particular question has not been studied systematically.1
