To the Editor:—  With great interest we noted the article of George Merrill in the May 22 issue of THE JOURNAL entitled "Neostigmine Toxicity: Report of Fatality Following Test for Myasthenia Gravis." To emphasize the advisability of awareness of the untoward reactions attendant on the use of neostigmine, we call attention to observations on patients in whom pulmonary edema developed after treatment with therapeutic doses of neostigmine orally in February 1947. Our report, in which we discussed the implications of the connection between neostigmine and pulmonary edema, was accepted for publication by Annals of Internal Medicine in December 1947.
