The authors have certainly put forth an attractive hypothesis. However, in the two patients studied most extensively by Sutherland et al (Acta Paediatr Scand 201 (suppl):1, 1970), an important finding was the inability of the children to conserve serum magnesium. A further observation in each of our four patients was immature glomeruli in the renal periphery. Probably, more than one type of Bartter syndrome may exist, as it is difficult to explain all our findings on the basis of the hypothesis of Drs Kurtzman and Gutierrez.
