Dr Salinger's observations are retrospective. While we concur that careful history-taking and integration of historical facts into the analysis of the case is one of the cornerstones of good medical practice, we question that he would have reached the diagnosis of obstructive uropathy sooner in either case, but particularly in the second. It is rewarding nevertheless that Dr Salinger did bother to read the case histories.
