Low-power examination of a cutaneous punch biopsy specimen revealed moderate epidermal hyperplasia overlying superficial dermal and middermal noncaseating granulomas. The granulomas were surrounded by small numbers of lymphocytes. Special stains for organisms revealed rare, thin, intracellular acid-fast bacilli (Figure 3, arrows). Tissue culture identified the organism as M avium-intracellulare.
