In Reply We thank Dr Klonizakis for his supportive comments with respect to our systematic review of randomized clinical trials on the effects of adding exercise to compression therapy for treating venous leg ulcers.1 While more suitably qualified experts might care to comment on his observations about microcirculatory dysfunction, we concur with the need for a multifaceted exercise regimen in this patient group, as well as the need for definitive trials.
