Contact Allergy to Tolnaftate

A 43-year-old man developed delayed hypersensitivity to tolnaftate. Patch testing indicated sensitivity to tolnaftate, but not to the preservative or vehicle of the proprietary solution. There have been no other instances of tolnaftate sensitivity reported in the literature, despite its use by several million persons in the past decade. Perhaps prospective epidemiologic studies may ascertain if this is a rare isolated sensitization or if any other instances have been missed.
