About 20% of hospitalized Medicare patients are readmitted within 30 days.1 Several randomized controlled trials (RCTs)2-4 have shown that it is possible to reduce hospital readmission rates through improved discharge planning and coordination and posthospitalization care. Decreasing hospital readmissions offers the hope of improving care while simultaneously reducing health care costs.
