We read with great interest the letters from Drs Gupta, Hayreh, and Krakoff and colleagues, and we think that our conclusions are not at odds with their concerns. In fact, our results1 support the hypothesis that IOH is not a totally benign condition; therefore, we conclude that patients with IOH "and demonstrated cardiovascular remodeling" deserve lifestyle modifications and longitudinal monitoring to establish the actual need for drug treatment.
