To the Editor In a recent issue of JAMA Internal Medicine, Agar et al1 published the results of a double-blind randomized clinical trial highlighting the treatment of distressing delirium symptoms in patients receiving inpatient hospice or palliative care services. This study is unique because it is a placebo-controlled study of an inpatient palliative care population requiring care by a specialist palliative care team. The size of the study is also adequately powered, which is beneficial when compared with prior randomized clinical trials addressing treatment of delirium with antipsychotics. However, there are several treatment issues and points worth discussion.
