In Reply We thank Dr Avasarala for his interest in our report1 and his thought-provoking comments. As pathologists, we often have the benefit of hindsight. The correct diagnosis, which may have eluded our clinical colleagues for months, may be immediately obvious to us when we look at a biopsy under the microscope. However, in this particular case, we find it difficult to fault the clinicians who initially saw this patient at an outside institution.
