Harold G. Wolff had completed the second edition of his remarkable book shortly before his untimely death. It appeared in 1963 and like the first edition (1948) became the reference that was turned to for authoritative information about head pain by those in any field dealing with the ubiquitous symptom. The time has not yet arrived to discard this second edition.
For someone other than the author to revise what has become a medical classic requires temerity. It can hardly be hoped to continue in the vein of the original or to match the style exactly. However, what can be achieved is to interweave the developments, in this instance those of about a decade of considerable activity. As Wolff had written in the preface to the second edition: "The search continues and is spreading, and it is likely that the next dozen years will yield many answers."
