Dementia in Idiopathic Hypoparathyroidism: Rapid Efficacy of Alfacalcidol

• A man with a three-year history of profound dementia and convulsions was found to have idiopathic hypoparathyroidism. Twenty-four hours after the oral administration of alfacalcidol (1α-hydroxycholecalciferol), a virtually complete restoration of mental function was seen. Normal plasma calcium levels were only achieved after six weeks of therapy. It is suggested that reversion of a metabolically blocked neuronal state could take place rapidly through activation of calcium-cell receptors by alfacalcidol-promoted rising ionic calcium levels.
