We have reviewed the interesting comments of Franciotta et al, who propose that MS and NMO may be distinguished by biologic differences in their respective humoral immune responses: oligoclonal banding, anti-aquaporin-4 (AQP-4) specificity, and background polyreactivity. Given the differing views of the authors1-3 on the distinctness of NMO and MS, as expressed in our recent “Controversies in Neurology” series, understandably we will express somewhat disparate responses to these comments.
