The recent article published by Pittock et al1 proposes that the association between neuromyelitis optica (NMO) spectrum disorders and other autoimmune diseases such as Sjögren syndrome (SS) or systemic lupus erythematosus represents 2 independent conditions that coexist in the same patient. We describe a patient with clinical, serologic, imaging, and neuropathologic findings that support their notion; our patient underwent anti-aquaporin 4 antibody testing that was found positive at a dilution of 1:1920.
