To the Editor Mansberger et al1 have written an interesting article comparing telemedicine with traditional examinations for usefulness in diabetic retinopathy screening. Their results indicate telemedicine screenings are cost-effective and baseline levels of diabetic retinopathy remain constant over time. It is notable that the authors do not emphasize 2 issues that may have bearing on the usefulness of telemedicine studies.
