A woman in her early 30s presented with recent onset of floaters in the right eye. Examination revealed a highly motile filarialike worm in the epiretinal area, with no sign of intraocular inflammation (Figure). Pars plana vitrectomy was performed, and, after removing the internal limiting membrane overlying the worm, the worm was removed manually via an extrusion cannula connected to a syringe. The next day, the patient’s symptoms had resolved, maintaining her 20/20 OD visual acuity.
