Reply to Drs. Hirose and Schepens

—We are indeed pleased that Drs. Hirose and Schepens have made comments stimulated by our paper, Giant Tear Dialysis in the Outer Layer of Retinoschisis (Arch Ophthalmol 88:336-340, 1972).Their speculation that increased inner pressure of the schisis cavity "ruptured" the outer wall seems like an interesting and valid explanation especially since they have observed the formation of these "breaks" in the outer wall of the schisis cavity. Possibly this theory could also explain a previously reported case. In 1946, Duke-Elder1 reported a case of an enlarging peripheral retinal cyst which disappeared overnight and in its place a dialysis and flat detachment was noted. Drs. Hirose and Schepens' theory could explain Duke-Elder's case, assuming that the cyst ruptured at the ora serrata and resulted in a dialysis and detachment.The semantics in ophthalmology is a difficult problem and our decision
