The patient in this case presented with chronic obstruction of the LDS, which has a broad differential diagnosis, including granulomatous inflammation, granulomatous infection, dermoid cyst, mucocele, lipoma, lymphangioma, squamous cell carcinoma, adenoid cystic carcinoma, hemangioma, sarcoma, melanoma, lymphoma, and secondary involvement of a systemic lymphoproliferative disorder.1The final diagnosis in the index case was systemic CLL/SLL with involvement of the LDS, ethmoid labyrinth, and nasal septum.
