IN MARCH 1959, I proposed a new audiosurgical system, sonoinversion.1 In sonoinversion, the sound wave is channeled toward the round window, which becomes the active window, while the oval window, or surgical opening which substitutes it on the scala vestibuli (fenestration or stapedectomy) is limited to act as a passive counterfenestra. This new audiosurgical system is applied to certain cases of tympanoplasty where the type of lesion makes it difficult to channel the sound wave toward the oval window.
It is also quite possible to attain hearing recovery up to the practical level in cases of otosclerosis with a good sensorial reserve. For this purpose, the round window is exteriorized by the transportation of the posteroinferior quadrant of the tympanum. Thereupon, a passive fenestration is performed. In this way it is possible to perform "closed" fenestrations, which have the advantage of minimizing postoperative care. Sonoinversion has been established as an
