The presence of a very fine ocular dysmetria can be detected as a calibration overshoot during the performance of routine electronystagmography (ENG) examinations.

This overshoot of the standard ten degree fixation target arc is an excellent sign of a cerebellar defect. We reviewed 2,248 consecutive ENGs. Of 40 overshoots described, 33 patients had confirmed cerebellar defects, five had no confirmed cerebellar disease, and two had no follow-up. Correlation with clinical cerebellar signs and other ENG abnormalities is presented.
