We assume that this letter refers to the sentence on page 608 and we wish to thank Dr Johnson for reminding us that descriptive terms usually have precise definitions. In this instance it would have been more helpful had the author chosen to describe exactly the clinical condition under consideration. Figure 4 seems to illustrate the point he was making.
