Controversy remains regarding the optimal surgical approach for the treatment of rectal cancer. Achieving an intact total mesorectal excision and negative circumferential and distal margins are primary goals of surgery. But the results of recent randomized clinical trials suggest that laparoscopy may not be an adequate means to this end.1,2
