To the Editor We read with interest the article in JAMA Surgery by Smits et al1 and accompanying Editorial by Rangelova et al2 on management of severe pancreatic fistula following pancreaticoduodenectomy. Although it is appreciated that percutaneous drainage is widely available as a nonoperative management strategy, we were quite surprised by the near complete omission of endoscopic therapy as a management strategy.
