In Reply.—Dr Finer is correct. Congenital absence of the pulmonary artery is an element of the differential diagnosis in the unilateral hyperlucent lung. Reduced perfusion of the affected side is seen in these cases and is, as he says, usually a reflection of pulmonary artery hypoplasia.
