Errors in the Author Affiliations and Corresponding Author Address

In the Original Investigation titled “Survival Benefit of Breast Surgery for Low-Grade Ductal Carcinoma In Situ: A Population-Based Cohort Study” published online June 3, 2015, in JAMA Surgery (doi:10.1001/jamasurg.2015.0876), errors appeared in the Author Affiliations and Corresponding Author address. Stephanie Wong, MD, should be affiliated with Harvard School of Public Health instead of Harvard Medical School. Yasuaki Sagara, MD, should have Harvard Medical School listed as his affiliation in the Corresponding Author address instead of Harvard School of Public Health. This article was corrected online.
